The document lacks a document title. This means that various auxiliary tools (e.g. screen readers) automatically use the file name, which is often unclearly worded.

As soon as you save the document and check it again with the PDF Accessibility Checker, the error message No title available in the XMP metadata disappeared.
When saving, make sure to give the document a new name (e.g. Document Title_Rev2) and not oversave the previous version of the document. This way, you can go back to the previous version of the document if something goes wrong.
Tips for Error Prevention
Create your document using a standard word processing program such as Microsoft Word, as you can set a document title in such programs (in Word, for example, under File > Information > Properties). This will then be correctly entered into the PDF during the PDF export, so you do not have to correct the error afterwards.
